Valproic acid is a type of drug that is primarily used in the management and treatment of seizures and epilepsy. Most of the Valproic acid bounds to the blood proteins and certain amount circulates unbounded (“free”). Only the “free” fraction has a therapeutic effect. By measuring the ratio between “free” and “total” amount of Valproic acid, doctors can assess if the correction of a drug dose is needed. People who are diagnosed with epilepsy require lifelong treatment. They need to maintain a tight amount of medication in the blood at all times. Sometimes, even while on medication, patients have breakthrough seizures. This calls for a test to measure the levels of the valproic acid in the blood to see if there is a therapeutic amount in the bloodstream. This helps to guide treatment or find alternative causes for breakthrough seizures. It is also helpful if the patient exhibits signs of toxicity from a dose that it too high.
While a regular Valproic Acid blood test is more common, some people may find it helpful to measure Free Valproic Acid. Free Valproic Acid is not bound to proteins in the blood and is thus active and available for the body to use.
